Registration Renewal 2018 

Registration Renewal for 2018 is now closed. However, if you have not yet submitted your Information Return and have received Notice, you can click the link below to submit the form. Please be reminded that an administrative fee has been applied to your cart. See below for instructions on how to pay.

Both the Information Return and the administrative fee must be received by no later than May 8, 2018.

Renewing your registration is a two-step process:

  1. Paying the annual fee; and
  2. Completing your Information Return.

Under section 18 of the College by-laws, all Members of the College are given 45 days’ notice that annual registration fees are due.

You will be able to pay your 2018 registration fees online after submitting your Information Return, or at any time by clicking on the cart on your Account Page. Note that the College must receive both your payment and the Information Return form in order for your registration to be successfully renewed. 

The online payment system was accessible until 12:01 am EST on April 1, 2018. A late fee of $275 + HST was automatically applied to the Carts of those who have not yet paid their registration fee.

You may choose to pay by cheque, money order or bank draft, please ensure that your payment is received by the College before 5:00 p.m. EST on May 8, 2018. For efficient processing, include your registration number on the cheque or money order.


Payment Plan Program 

The College initiated a Payment Plan Program as a pilot this year for Members to pay their registration fee in installments rather than in one lump sum payment. March 23, 2018 was the deadline for interested Members to apply for participation in the Program.

Completing your Information Return 

The College also collects certain information from its Members as required under the Registration Regulation and to meet the reporting requirements of HealthForceOntario. Details such as one's practice hours will be asked annually. Certain other previously collected information may be used to pre-populate the online Return each year. 

Members who did not complete the Information Return by the deadline have been sent a Notice of Failure to Provide Information, and will be provided 30 days in which to submit the data or be suspended. This notice also carries a $50 (+HST) administrative fee.

If you received a Notice from the College on April 3, 2018, the final deadline for submitting the Information Return is May 8, 2018 at 11:59 pm EST.

To pay the administrative fee, please click on your Cart in the top right-hand corner, and follow the prompts to pay. You will receive an Order Confirmation email immediately upon payment. If you do not receive such confirmation, please contact the College at your earliest opportunity to check if payment has been received.

  • A calculator;
  • Your scheduler or appointment book (to provide practise hour information);
  • Contact information for all practice locations;
  • Professional liability insurance coverage certificate.
  • Any information and documentation about findings of guilt or current proceedings.
